Ice pick scars are deep, narrow, V-shaped pits in the skin, usually under 2mm wide. They look like a sharp point was pressed in and left a tiny tunnel behind.

They are one of the three atrophic acne scar types, alongside rolling and boxcar scars, and they are the hardest to treat. The damage runs deep into the skin, not just across the surface.

At Skinology we see them most on the cheeks, temples and nose. Here is an honest, research-backed look at what causes them and what genuinely helps.

What causes ice pick scars?

Inflamed acne is the typical reason. An acne cyst or nodule will damage the skin and the underlying tissue, and rather than heal flat, it will pull in on itself through a channel and become a deep indentation. They can also be caused by chicken pox, although not nearly as often as by acne.

Can you prevent ice pick scars?

For the most part, yes, through early treatment of deeper acne. Don’t allow your cystic or nodular acne to scar, and don’t ever pop an inflamed pimple. Popping is one of the quickest routes from a breakout that should leave no scar to a scar forever left behind.

Why are ice pick scars on the nose and cheeks harder to treat?

The skin on the nose is thinner with less support underneath, so this area needs a more careful, conservative approach. Cheeks tend to scar in clusters and sit in full view, which is usually why patients want them sorted first.

Does filler work on ice pick scars?

Not well for true ice pick scars. Filler lifts shallow, wide dips like rolling scars, but ice pick channels are too narrow and too deep for it to fill properly. In mixed scarring it may still help the rolling scars, just not these.

Does microneedling work on ice pick scars?

On its own, not much. Microneedling is excellent for overall texture and for rolling and some boxcar scars, but the needles rarely reach the depth of an ice pick channel. It works as a supporting step, not the main fix. Anyone selling microneedling alone as a full ice pick solution is overpromising.

What actually works: TCA CROSS and punch excision

These two have the strongest evidence for true ice pick scars.

TCA CROSS: a precise drop of high-strength acid is placed into the scar channel itself, rebuilding it from the inside. Studies show four to six sessions, two weeks apart, give good to excellent improvement in most patients.

Punch excision: for deeper or wider scars, we remove the pit with a tiny punch and close it so it heals as a small flat line. Laser resurfacing a week later blends it further. It swaps a deep scar for a much smaller one rather than erasing it.

Laser resurfacing works best as a finishing step, not on its own. Most real results come from combining two of these, since scarring is rarely one single type.

Is RF microneedling safe for ice pick scars?

Be careful with the hype. In October 2025 the FDA warned about RF microneedling after reports of burns, permanent scarring, fat loss and nerve damage. It can help in experienced hands, but it is a medical procedure, not a quick add-on, and results for true ice pick scars are mixed. The provider’s skill matters more than the device brand.

Are these treatments safe for darker, Pakistani skin?

This is the question that matters most locally. Brown and darker skin tones are more prone to post-inflammatory hyperpigmentation, the dark marks that can follow a procedure.

TCA CROSS works well on darker skin, but it needs proper priming, a sensible acid strength, and strict sun protection afterward. Punch excision suits all skin tones. The real safeguard is a provider who plans around how your skin heals.

Do creams or home remedies fix ice pick scars?

No home product closes a true ice pick scar. Retinoids, vitamin C and gentle acids improve general tone and texture and are good for everyday skin health, but they cannot rebuild a deep channel. If a cream claims to remove ice pick scars at home, treat it as a red flag.

What results are realistic?

Clear improvement is realistic. Complete removal is not something we promise. With the right plan matched to your actual scars, ice pick scars can become noticeably shallower and less obvious. The first step is identifying your scar types correctly, so sessions are not wasted on the wrong technique.

How do I get rid of ice pick scars?

TCA CROSS and punch excision, done in a clinic and often combined, are the evidence-backed options. Creams and home remedies cannot close the scar.

Treat deep, cystic acne early and never pick inflamed breakouts. Both sharply lower the chance of permanent scarring.

Neither is the main answer. Both do better on rolling scars. TCA CROSS and punch excision have the strongest evidence here.

Yes, with the right provider. Darker skin needs priming, a sensible acid strength and strict aftercare to control pigmentation.

Not completely, but significant improvement is realistic. TCA CROSS shows good to excellent results in most patients after a full course.
